29 CFR 1910.134(g)(1): Facepiece seal protection.  29 CFR 1910.134(g)(1)(i): The employer shall not permit respirators with tight-fitting facepieces to be worn by employees who have:  29 CFR 1910.134(g)(1)(i)(A): Facial hair that comes between the sealing surface of the facepiece and the face or that interferes with valve function;    On or about August 18, 2017, employees had facial hair that interfered with the sealing surface of the respirator facepiece during required respirator use. The employer did not assure that employees were clean shaven, or that facial hair was not between the face and respirator sealing surface.

On or about April 20, 2017, the employer had not developed and documented specific procedures for control of hazardous energy during servicing and maintenance activities. Employees performed maintenance and servicing activities covered by this part without the benefit of specific procedures for equipment, such as, but not limited to, routine and regular bearing replacement on the Wilson and S/A transloaders.
