29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a)Jensen Framing & Remodeling, Inc. at 9201 W. Kingfisher Circle, Sioux Falls SD, 57106:  On and prior to April 17, 2017, the exposing employer did not ensure each employee on a walking /working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Employees were installing roof structural members while standing on the wood structure without the use of a fall protection system. This condition exposed the employees to a fall hazard of approximately 9ft.

29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    (a)Jensen Framing & Remodeling, Inc., at 9201 W. Kingfisher Circle, Sioux Falls SD, 57106:  On and prior to April 17, 2017, the exposing employer did not ensure each employee working on a structural wood framed system were protected from potential falling debris from the roof above.  Employees were working on a lower level of the single-family home while roofing work was being conducting above. This condition exposed employees to falling debris, tools and materials.
