On or about June 23, 2017, the employer failed to provide abatement certification for this citation within 10 calendar days after the abatement date, exposing employees to physical hazards.  Windy City Catastrophe Solutions, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.20(b)(2), which was contained in OSHA inspection number 1226947, citation 2, item number 001 and was affirmed as a final order on May 24, 2017, with respect to a workplace located at 380 John Anderson Drive, Ormond Beach, FL 32176.

On or about January 25, 2017, roof area, back side of roof, employees working near two skylights were not protected from falling 12 feet through the (2' x 4') skylights to the floor below by a personal fall arrest system, covers or a guardrail system around each skylight.
