29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the Fong Ho Circular Cold Saw labeled as machine #245 did not have guarding to protect the unused portion of the saw blade.       b) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the Barker milling machine did not have guarding to protect the blade and belt pulley system.      c) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the band sander in the table did not have guarding to protect from moving/rotating parts.      d) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the Bliss C-45 labeled as #169 did not have point of operation guarding to protect the employee.      e) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the saw labeled as number #109 in the commutator area did not have guarding in place to protect the unused portion of the saw blade.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:     a) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the Fong Ho Circular Cold Saw labeled as machine #245 the point of operation guarding did not prevent employees from accessing the danger zone. The height and width of the opening is larger than parts being processed.      b) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the press labeled as #4 point of operation guarding did not prevent employees from accessing the danger zone. The adjustable barrier guards did not prevent access to the point of operation. The adjustable barrier guards protected the front and sides of the point of operation, but not the top.      c) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the press labeled as #13 point of operation guarding did not prevent employees from accessing the danger zone. The adjustable barrier guards did not prevent access to the point of operation. The adjustable barrier guards protected the front and sides of the point of operation, but not the top.

29 CFR 1910.219(b)(1): Flywheel(s) with parts seven feet or less above floor(s) or platform(s) were not guarded in accordance with the requirements specified in 29 CFR 1910.219(b)(1)(i) through (b)(1)(iv)(a):    a) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the press labeled as #4 did not have guarding in place over the flywheel.      b) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the press labeled as #13 did not have guarding in place to protect from the flywheel.      c) Eureka Electrical Products, Inc. / 79 Clay St., North East, PA: On or about April 19, 2017 the saw labeled as number #109 in the commutator area, the guard for the upper and lower flywheels was broken in several areas exposing moving parts. Additionally, the guard on the top did not cover the lower section of the flywheel.
