29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:   a) 160 Maplewood Ave., Maplewood, NJ   Laborers working in an excavation were exposed to struck-by and crushing injuries in the up to 7 feet deep trench which was not adequately sloped or protected by shields or shoring.
