29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: a) On or about April 13, 2017, at the above addressed jobsite - employees performing window installations on the second floor were exposed to fall hazards due to unprotected window/wall openings, thereby exposing the employees to falls of the approximately fifteen (15) feet. b) On or about April 20, 2017, at the above addressed jobsite -employees performing window installations on the third floor were exposed to fall hazards due to unprotected window/wall openings, thereby exposing the employees to falls of the approximately thirty (30) feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) On or about April 13, 2017, at the above addressed jobsite - the employer was not providing adequate information regarding fall hazards to employees performing window installations working at heights over six (6) feet, thereby exposing the employees to falls hazards.
