29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    Side of building, north side of building:  On or about April 24, 2017, an employee was exposed to a fall hazard greater than 10 feet to the concrete ground below while performing demolition working from a Skyjack scissor lift(model SJ111 3219), while standing on the midrail and toe board of this lift.

29 CFR 1926.850(g): Wall openings in a structure being demolished were not protected to a height of approximately 42 inches:     In the alternative     29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     Roof of building, north side: On or about April 24, 2017, employees were exposed to a fall hazard greater than 6 feet from the edge of the roof to the concrete ground below, while demolishing a section of the exterior brick wall.
