29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    129 Elm Street, Hatfield, MA: On or about April 28, 2017 the employer had not established a program of an energy control procedure, employee training, and periodic inspections to ensure that before any employee performed any servicing or maintenance on the compactor compartment of recycling trucks, bulk dumpster delivery trucks and side load compacting garbage collection vehicles where the unexpected energizing, startup or release of energy could occur and cause injury exposing employees to crushing and amputation hazards.    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    129 Elm Street, Hatfield, MA: On or about April 27, 2017 one or more methods of machine guarding was not provided or maintained to protect the operator and other employees in the machine area of the compactor body for a Lodal EVO-MAG-20CN  serial#149AF55BOJK006676,side loading recycling truck, exposing employees to caught between hazards.  The anti-retract limit switches were overridden allowing the compaction ram to be overextended and still retract in the automatic position exposing employees to caught between hazards from the back side of the ram and hydraulic ram access panel lid.    a) Lodal EVO MAG 20CN, #646, Anti-retract switch tied back with plastic cable ties (zip ties).  b) Lodal EVO MAG 20CN, #640, Anti retract switch tied back with plastic cable ties (zip ties).    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
