29 CFR 1910.134(c)(2)(i): The employer determined that any voluntary respirator use was permissible, and the employer did not provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard"):  a)  Worksite, On or about April 21, 2017 - Employees wore N95 respirators while empting the dust collector, but were not provided Appendix D of the standard.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met:  a)  Worksite, On or about April 21, 2017- T.K. Plastics's written hazard communication program was not specific and updated to describe criteria specified such as forms of warning labels on products and safety data sheets presence.    Employees uses hazardous chemicals such as but not limited to: cutting oil, methyl alcohol, phenolic and rubber dust generated from grinding and fumes generated from thermal decomposition.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with either the information specified under paragraphs (f)(1)(i) through (v) of this section for labels on shipped containers or product identifier and words, pictures, symbols, or combination thereof:  a)  Worksite, On or about April 21, 2017- Bottles of cutting oil and methyl alcohol & isopropyl alcohol were not labeled with product identifiers such as product name and hazard warnings.
