29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Location: a) Supported scaffold on southwest side of the building:    On or about 05/01/17, employees were cement facing on the fourth floor platform of a 10-tier-high supported scaffold, and were not protected from falling approximately 35 feet to a lower level.

29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system.    Location: a) On southwest side of the building:    On or about 05/01/17, employees were preparing to install window next to a wall opening on the fifth floor, and were not protected from falling approximately 45 feet to a lower level.
