29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.   Seymour Johnson AFB, Community Center: Employees had no means of safe access and egress working from a frame scaffolding.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   Seymour Johnson AFB Community Center: On or before May 2, 2017, an employee was working from a fabricated frame scaffold 12 feet high, and did not have the guardrails installed.

29 CFR 1926.451(g)(4)(vii): Each toprail or equivalent member of a guardrail system was not capable of withstanding, without failure, a force applied in any downward or horizontal direction at any point along its top edge of at least 100 pounds (445 n) or guardrail systems installed on single-point adjustable suspension scaffolds or two-point adjustable suspension scaffolds, and at least 200 pounds (890 n) for guardrail systems installed on all other scaffolds.    Seymour Johnson AFB Community Center: Scaffold guardrail system was not adequately secured with a double wrap wire connection. Single wrap wire tensile strength did not provide support for 200 lbs in any outward or downward direction.  Wire connections were not twisted to a minimum of 5 wraps to ensure the ultimate strength.
