29 CFR 1926.651(b)(2): Utility companies or owners were not contacted within established or customary local response times, advised of the proposed work, and asked to establish the location of the utility underground installations prior to the start of an actual excavation: Jobsite: Employees were preparing to place a 12,000 gallon sea water holding tank in the ground. The utility company was not notified of the proposed work and did not establish the location of underground utility lines. During the excavating employees undermined the underground electrical lines, which fell, causing the lines to separate from the transformer, exposing employees to electrical shock hazards.

This exposed employees to cave-in and struck by hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). Employees were working in an excavation that was approximately fifteen (15) feet deep making preparations for the installation of a 12,000 gallon sea water storage tank without the use of cave in protection.
