29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.    a)    Front of residence:  On or about August 3, 2014, and at times prior, the employer exposed employees to fall hazards of approximately 20 feet from the box attached to a boom lift, in that personal fall arrest was not provided to employees while installing flashing.

29 CFR 1926.602(c)(1)(vii): Unauthorized persons were allowed to ride on a powered industrial truck:   a)    Front of residence:  On or about August 3, 2014, and at times prior, the employer exposed employees to fall hazards from a scrap box attached to a boom lift, in that no safe place was provided to employees while being elevated and transported in a scrap box.
