29 CFR 1910.1200(h)(3)(i): The employee training did not include the methods and observations used to detect the presence or release of a hazardous chemical in the work area:  a) Worksite at 205 Enterprise Drive, Rockaway, NJ and company headquarters at 128 Berger St., Wood Ridge, NJ.  Workers cutting concrete flooring indoors with a gasoline powered saw did not observe the hazardous buildup of dust, smoke and exhaust within the room which set off a smoke alarm.  No devices were used to monitor carbon monoxide.  Violation occurred on or about 4/25/17.

29 CFR 1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  a) Worksite at 205 Enterprise Drive, Rockaway, NJ and company headquarters at 128 Berger St., Wood Ridge, NJ.  Workers dry cut concrete flooring indoors with a gasoline powered saw, setting off a smoke alarm with fire department response and building evacuation.    Violation occurred on or about 4/25/17.
