29 CFR 1926.20(b)(2): It shall be the responsibility of the employer to initiate and maintain programs as may be necessary to comply with accident prevention; such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a)  On May 4, 2017 at E4120 Runway Drive in Waupaca, the employer did not conduct adequate safety inspections prior to employees working on barn soffit from scaffold at 10.5 ft. above surface level.  No fall protection equipment installed, work platform not fully decked and positioned greater than 14 in. from face of barn wall, and base plates used as foundation on grassy surface.

29 CFR 1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).  a)  On May 4, 2017 at E4120 Runway Drive in Waupaca, employees were working from one 20 ft. Long scaffold plank supported by frame scaffold at 10.5 ft. above surface level without the platform fully decked between the scaffold uprights.

29 CFR 1926.451(b)(3): Except as provided in paragraphs (b)(3)(i) and (ii) of this section, the front edge of all platforms shall not be more than 14 inches (36 cm) from the face of the work, unless guardrail systems are erected along the front edge and/or personal fall arrest systems are used in accordance with paragraph (g) of this section to protect employees from falling.  a)  On May 4, 2017 at E4120 Runway Drive in Waupaca, employees were installing soffit on a barn eave from a scaffold plank at 10.5 ft. above surface level with the plank edge positioned 17 in. from the face of the barn wall.

Long stage plank supported by frame scaffold at 10.5 ft. above surface level without utilizing any fall protection equipment.
