On or about May 04, 2017, At 709 East Main St., Lockport, LA, the employer did not effectively determine proper protective equipment (PPE) for employees working in areas where electrical and overhead hazards were present.  Employee(s) worked near hazards including, but not limited to, damaged overhead electrical power lines, low hanging energized power lines, broken wooden electrical poles, and transformers without wearing adequate PPE.
