Location - 121 Donald Lynch Blvd, Marlborough, MA 01752. On or about May 7, 2017, employees were exposed to a fall hazard of greater than twenty feet in height to lower level while being attached to horizontal lifelines (two separate lines) that were not designed by a qualified person or manufactured for lifeline/fall protection use. These two lifelines did not maintain a safety factor of two.

On or about May 7, 2017, employees were not protected from falling greater than twenty feet to lower level while using a "3/8 DIA Vertical Choker Basket" used as a horizontal lifeline that was not protected against being cut or abraded. The line was wrapped around the sharp edges of an I-beam then attached to roof anchors.
