On or about May 5, 2017 in the maintenance shop, lockout and tag out procedures were not developed, documented, or utilized where employees work on forklifts and battery chargers, employees are exposed to hazards to include, but not limited to :  a.  a crushing hazard when working at removing batteries and changing tires  b. a freezing hazard when changing propane cylinders when hoses have to be repaired or replaced  c. electrical hazards of up to 220V when working on the battery chargers

On or about May 5, 2017 in the maintenance shop, the employees of National Maintenance Services and those of DHL Supply Chain were not informed of each others lockout and tag out procedures.  Employees are exposed to crushing, freezing, and electrical hazards while performing service and maintenance on equipment including, but not limited to forklifts and battery chargers.
