29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures:    JOB SITE: 210 Ashton Circle, Saint Louis, MO 63129, on or about 5/8/2017, workers were observed on top of an 8:12 pitch roof, approximately 9 feet 2 inches eave to ground level, actively installing new siding and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures.  The workers were exposed to a fall hazard.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:    JOB SITE: 210 Ashton Circle, Saint Louis, MO 63129, on or about 5/8/2017, workers were observed on top of the front overhang roof, approximately 9' 2" eave to ground level, used a KPro 8-foot, Type 1A, Fiberglass Step Ladder in a closed position to install an attic vent.  A second worker on the roof was holding the ladder to prevent the ladder from shifting.  The workers were exposed to a fall hazard.
