29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) New residential construction on Lot #3, Front porch roof: On or about May 9, 2017, and at times prior, the employer exposed employees to struck-by hazards while operating a pneumatic nail gun at 120 PSI, in that protective eyewear was not provided while engaged in framing activities.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) New residential construction on Lot #3, Front side at work bench: On or about May 9, 2017, and at times prior, the employer failed to inspect the electrical cords prior to the start of the shift, in that cords used had damaged protective casing and a damage strain relief exposing the internal copper circuits as employees used this cord to power the circular saw used to cut rafters.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   (a) New residential construction located on Lot #3, Top plate of front porch: On or about May 9, 2017, and at times prior, the employer exposed employees to fall hazards of 11-feet, 6-inches to the lower level, in that a fall protection system was not provided to the employees installing roof rafters.   (b) New residential construction located on Lot #3, Roof: On or about May 9, 2017, and at times prior, the employer exposed employees to falls of over 20-feet from a 10/12 pitched roof, in that a fall protection system was not provided to employees engaged in framing work.

29 CFR 1926.1052(c)(1): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with at least one handrail and one stairrail system along each unprotected side or edge:  (a) New residential construction located on Lot #3, First floor stairway: On or about May 9, 2017, and at times prior, the employer exposed employees to fall hazards of up to 4-feet to the lower concrete surface, in that stair rails were not installed along the open sides of the stairway used by employees to access the upper levels.
