29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about May 9, 2017, the employer failed to provide and ensure the use of adequate personal protective equipment to employees who clean the dust collection room. Employees are required to clean combustible dust and were not provided fire-resistant clothing.

29 CFR 1910.1200(h)(3)(ii): Employee training did not include the physical and health hazards of the chemicals in the work area:  On or about May 9, 2017, the employer failed to adequately train employees in the physical hazards related to combustible dust. Combustible dust was collected in the dust-collection room immediately adjacent to the Time Saver equipment. Employees cleaned the dust/room using non-approved tools and inadequate PPE. Employees were exposed to a fire/explosion hazard.
