An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    (a) Chicago Die Cutting Press 770: On or about May 10, 2017 the mechanical press was not turned off or shut down using the procedures established for the press prior to the press operator setting a die.  This exposed the press operator to crushing hazards and amputation injury.

(a) In the warehouse, a 24 volt battery charger, bolted onto a pallet was connected/direct wired with a flexible cable to the side of a 3 phase 480 volt AC breaker switch box.
