29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:     On or about May 11 2017, in the rigging shop, four compressed gas cylinders were not properly secured or supported to prevent the cylinder from falling, thereby exposing employees to a struck by hazard.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:     On or about May 4, 2017, in the Sawmill building, a Craftsman 1/4 HP abrasive wheel bench grinder, model #39719400 was not provided with a side guard on the right side of grinder exposing employees to rotating parts such as flange, nut and spindle.  Employee's used the bench grinder to clean threads with a wire wheel there by exposing the employee to a caught in hazard.

On or about May 11, 2017, the employer did not develop and implement an adequate fire safety plan that covered all of the actions necessary to ensure employees safety in the event of a fire. Significant fire hazards include but are not limited to: flammable gas, hazardous chemicals, fuel vapors from boats in storage, storage of marine batteries, and combustible materials.
