29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) West side of the building, on or about May 10, 2017, an employee was elevated to the roof with a work platform that was supported by a Genie GTH-636 #0808 high reach forklift, exposing the employee up to a 17-foot fall hazard.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: a) At the job site, on or about May 10, 2017, an employee was observed working from the roof removing and replacing metal sheeting onto the wooden roof deck without any means of fall protection, which exposed the employee up to an 8-foot fall hazard. b) West side of the building, on or about May 10, 2017, two employees were observed standing/working from the roof removing and replacing metal sheeting onto the wooden roof deck without any means of fall protection, which exposed the employees up to a 17-foot fall hazard.
