29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a) Front/Building Exterior: An employee working without a hard hat were exposed to a struck by hazard from a nail gun used above his head,  on or about 5/11/2017.  b) Front/Building Exterior: An employee working without a hard hats were exposed to a struck by hazard from a Lull that was staging material on a wooden platform  on the roof,  on or about 5/11/2017.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) Front/Building Exterior: Employees working without any means of fall protection were exposed to a fall hazard of approximately 27 feet while engaged in framing activities, on or about 5/11/2017.   Express Construction 30 Corp aka Express Construction 4, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13, which was contained in OSHA inspection number 1069708, citation number 1, item number 1 and was affirmed as a final order on 11/30/2015, with respect to a workplace located at Karlsburg Rd, Monroe, NY 10950.  Express Construction 30 Corp aka Express Construction 4, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13, which was contained in OSHA inspection number 1125723, citation number 1, item number 1 and was affirmed as a final order on 7/13/2016, with respect to a workplace located at730 North Avenue, Union, NJ 07083.
