29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks: a) St. Marys Tool & Die, machining area: On or about May 11, 2017, the rotating chuck of the Shigiya grinder/lathe was not guarded to prevent employees from having hands near the rotating chuck of the machine during operation. b) St. Marys Tool & Die, machining area: On or about May 11, 2017, the rotating chuck of the Maximum Advantage Carolinas grinder/lathe was not guarded to prevent employees from having hands near the rotating chuck of the machine during operation. c) St. Marys Tool & Die, machining area: On or about May 11, 2017, the unused portion of the blade and the blade guide wheels on the Roll-In Vertical Band Saw were not guarded. d) St. Marys Tool & Die, machining area: On or about May 11, 2017, the unused sections of the blade and the blade guide wheels on the W. F. Wells horizontal band saw were not guarded.
