29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a) On or about 5/09/17, on the east side of a house at 137 Parkwood Ave., Kenmore, NY: Employees used an extension ladder to access the narrow aluminum pik on a  ladder jack scaffold that was approx. 18 ft. above the ground. The side rails of that ladder only extended approx. 20 inches above the pik.      NO ABATEMENT CERTIFICATION REQUIRED

No fall protection system was utilized. Pitch of the roof was approx. 12 in 12.       b) On or about 5/09/17, on the east side of a house roof at 137 Parkwood Ave., Kenmore, NY: Two employees engaged in roofing work on a steep roof were exposed to a fall of approx. 19 ft. 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   a) On or about 5/09/17, on the roof installation jobsite on a house at 137 Parkwood Ave., Kenmore, NY: Employees engaged in construction work including roofing were not trained to recognize potential hazards on this type of jobsite, including but not limited to falls from roofs, ladders, skylights, scaffolds, electrical hazards, unguarded tools/machines, fire hazards, and the need for personal protective equipment to protect them from various hazards. They also were not trained in the applicable OSHA regulations to control or eliminate these hazards.     ABATEMENT DOCUMENTATION REQUIRED
