The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.   a) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Grizzly 10 Inch Table Saw Model G0691 Serial Number 140180-17, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.    b) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Jet 10 Inch Table Saw Model JTAS-10XL-3 Serial Number 3063630, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.    c) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Powermatic Wood Shaper Model 27-SHAPER L/T Serial Number 0309027222, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.    d) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Powermatic 18 inch Cut Up Model COS-18L Serial Number 0303COS18L032, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.    e) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Jet Wood Shaper Model JWS-25CS Serial Number 3001174, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.    f) On or about May 10, 2017, and at times prior, the employer exposed employees to struck-by hazards in that employees were required to change blades on a Champ Fond Wood Shaper Model SP-102 Serial Number 102710, without any training provided, specific procedures developed, and periodic inspections performed on the unexpected energizing, startup or release of stored energy.
