29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  The employer is failing to ensure that all employees exposed to fall hazards while working on a commercial roof are utilizing fall protection.  This most recently occurred on May 16, 2017 on the jobsite located at 10901 Jaynes Plaza, Omaha, NE 68164.  Employees were observed on the roof of a 3-story apartment with a 6 in 12 steep sloped roof at heights greater than 6 feet above the ground and not utilizing any form of fall protection.

29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:   The employer is exposing employees to fall hazards associate with using faulty personal fall arrest systems (PFAS) while working on a commercial roof.  This most recently occurred on May 16, 2017 on the jobsite located at 10901 Jaynes Plaza, Omaha, NE 68164.  Employees are re-roofing an existing apartment building that has a 6 in 12 steep sloped pitch and a ground to eave height of about 30 feet.  The PFAS being used by employees has not been inspected as evident by the rusted connectors and frayed ropes.   Rusted connectors can cause them to break easily or cause cuts and tears to other components of the PFAS.  Frayed ropes can easily rip and tear apart in the event of a fall causing the employee to hit the ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  The employer is failing to ensure that each employee exposed to fall hazards has been effectively trained on fall hazard recognition and on how to use fall protection.  This most recently occurred on May 16, 2017 on the jobsite located at 10901 Jaynes Plaza, Omaha, NE 68164.  The employer has 7 employees who have not receive fall protection training and are exposed to fall hazards while re-roofing an existing apartment building that has a 6 in 12 steep sloped pitch and a ground to eave height of about 30 feet.
