Site - On or about May 17, 2017, employees engaged in roofing activities were not wearing fall protection and exposed to falls of over 19 feet.

Site - On or about May 17, 2017, the extension ladder in use by the employees to access the roof did not extend at least 3 feet above the upper landing surface.
