29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.       A. Pistons Roofing worksite located at 3530 Executive Parkway, Toledo, Ohio: On or about May 16, 2017, the employer did not provide employees with adequate fall protection while applying new roofing material on a 5/12 pitch commercial structure. Employees were exposed to a fall distance of approximately 28.00 feet.
