29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): (a) Throughout the facility - Energy control procedures utilized when maintaining and servicing machines and equipment were inadequate and did not fully address the scope, purpose, authorization, rules, the means to enforce compliance and the types and magnitudes of hazardous energy. During the period between May 1, 2017 and June 22, 2017, employee(s) were exposed to potentially hazardous energy sources when performing service and/or maintenance on machines in the workplace, including the Mazak milling machine. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices: (a) Grinding Department - Employees in the Grinding Department perform servicing activities to the grinders, including replacing the grinding wheels on the surface grinder, without locking and tagging out the equipment. On or about May 15, 2017, an employee performed a service activity on a surface grinder while setting up the machine for a new job. The machine was not locked and/or tagged out at the time of the service activity. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) In areas including the Grinding Department - On or about May 15, 2017, a worker changed the grinding wheel on a surface grinder and failed to implement energy control application steps as the machine was only shut down using the control circuitry (on/off switch) and not deenergized to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
