29 CFR 1915.73(c): The employer did not ensure that employees working around open hatches not protected by coamings to a height of 24 inches or around other large openings, the edge of the opening shall be guarded in the working area to height of 36 to 42 inches, except where the use of such guards is made impracticable by the work actually in progress.   a)Dry Dock Construction lot site:  On or about May 17, 2017 and at times prior, the employer exposed employees to fall hazards by failing to ensure employees used guardrails, safety net or fall arrest system  to prevent falls of over 8 feet into open holes and open hatches located on the dry dock boat being constructed.

29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2).  a)Dry Dock Construction lot site:  On or about May 17, 2017 and at times prior, the employer exposed employees to fall hazards by failing to ensure employees used guardrails, safety net or fall arrest system  to prevent falls of 8.53 feet from the walkway on the dry dock boat being constructed to the stationary dry dock landing below.

29 CFR 1915.75(b): Sides of gangways, Ramps, or permanent stairways, including those used for access to wing walls from dry dock floors did not have a railing with a midrail.   a) Yard 1 dry dock boat construction site:  On or about May 17, 2017 and at times prior, the employer exposed employees to fall hazards of approximately 10 feet by not ensuring the gangway from the land to the stationary shop dry dock had a mid-rail in place.   b)Yard 1 dry dock boat construction site:  On or about August 1, 2017 and at times prior, the employer exposed employees to fall hazards of approximately 31 inches by not ensuring the  fixed stairs from the dry dock to vessel had standard railing.
