29 CFR 1926.300(b)(2): Moving part(s) of equipment exposed to contact by employees or which otherwise created a hazard, were not guarded:    (a) New residential construction on Lot #20, East side of structure near retaining wall: On or about May 11, 2017, and at times prior, the employer exposed employees to amputation hazards while operating a Jenny Air Compressor, in that the belt guard provided was damaged and not covering the moving belts and pulleys as employees used this equipment to power framing nail guns used for framing activities.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) New residential construction, East gable side roof: On or about May 11, 2017, and at times prior, the employer exposed employees to fall hazards of 22-feet while working from a 8/12 pitched roof, in that a fall protection system was not provided to an employee installing 1/2-inch plywood sheathing.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a) New residential construction on Lot #20, East gable roof: On or about May 11, 2017, and at times prior, the employer failed to train employees to recognize fall hazards while working at heights over 6-feet on a roof or top plates, in that employees worked without fall protection systems while engaged in framing activities.
