29 CFR 1910.28(b)(3)(ii): Each employee was not protected from tripping into floor holes:      a) On or about May 17, 2017 and at times prior, employees were exposed to fall hazards from metal grates that were missing in between the rail tracks at the transfer tables. On May 4, 2017, an employee fell into a hole while working at the railroad tracks at transfer table #10.

29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements:      a) On or about May 17, 2017 and at times prior, employees were exposed to fall hazards from stairs located at the Transfer Tables Station having 4 risers were not equipped with stair rail systems and handrails.
