29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, Safety net systems, or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: (a) When an employee traversed from the cat walk to the top of the fuel tanks he was not protected from falling to the lower level. This exposed the employee to a fall hazard of approximately 11 feet 8 inches. (b) When an employee was at the tank cover and had his fall protection system fastened to the top of the fuel tank he was not protected from falling and contacting the lower level below. This exposed the employee to a fall hazard of approximately 11 feet 8 inches.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) A flexible extension cords outer protective cover had been pulled away from the the receptacle end of the cord. The extension cord was used to plug into the fuel trucks.
