29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    a) Foremost Manufacturing Co Inc., 941 Ball Ave., Union, NJ/Site Wide- The employer did not conduct an annual or more frequent inspection of the energy control procedures to ensure that they were properly implemented during the in service/lock out of equipment such as but not limited to the BB5 Polishing machines.    Observed on or about 5/22/2017.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by moving parts of conveyors and rotating parts:    a) Foremost Manufacturing Co Inc., 941 Ball Ave., Union, NJ/Site Wide- - The guard used for covering rotating parts was missing from the BB5-Polishing Machines exposing employees to amputation hazards.    Observed on or about 5/22/2017.
