On or about May 22, 2017 at the bottom seal bag machine where employees were exposed to electrical hazards where two dial indicators were recently taken out and the opening was not effectively closed.        b. On or about May 22, 2017 at the secondary panel of the bottom seal bag machine where employees were exposed to electrical hazards where a pressure gauge was recently taken out and the opening was not effectively closed.      c. On or about May 22, 2017 at the bottom seal bag machine under the name plate where employees were exposed to electrical hazards where an unused opening was not effectively closed.
