29 CFR 1926.405(a)(1)(i): Metal raceways, cable armor, and other metal enclosures for conductors were not metallically joined together into a continuous electric conductor and so connected to all boxes, fittings, and cabinets as to provide effective electrical continuity:  In the area of the site job-box a four-square electrical box containing a single duplex outlet attached to a metal cover. This cover was hanging loosely and not firmly attached to the above noted four square box. Employees were exposed to a shock hazard.

29 CFR 1926.405(a)(2)(ii)(H): A box was not used in temporary wiring when a change was made to a raceway system or a cable system which was metal clad or metal sheath:  In the area directly in front of the site job-box where the change was made the connections made with wire nuts were not protected by a box. Employees were exposed to a shock hazard.

In the area directly in front of the site job-box the metal four-square electrical box containing two duplex outlets did not have a cover installed. Employees were exposed to a shock hazard.
