29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice:    (a) On May 24, 2017, and at times prior, the employer exposed employees to slip and fall hazards in that wet and icy surfaces were not promptly cleaned up while employees were walking in the freezer and coolers to load and unload food products on the Crown Forklifts.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  (a) On May 24, 2017, and at times prior, the employer exposed employees to lacerations and cut hazards in that the written hazard assessment was not verified with the date it was performed and the signature of the person conducting the assessment.
