29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:   (a) 60 Hudson St. (West Broadway Side): On or about 05/23/2017, two establishment employees were exposed hazards when using portable a frame step ladders to install decorative exterior ventilation grates.  The ladders were used in the closed position and leaned against the building, exposing the employees to a potential fall to the concrete sidewalk below.

29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause them to lose balance and fall:   (a) 60 Hudson St. (West Broadway Side): On or about 05/23/2017, two establishment employees were exposed hazards when using portable a frame step ladders to install decorative exterior ventilation grates.  The employees were carrying the a large grate up the ladders, in tandem.  The grate had the potential to cause the employees to lose their balance, exposing them to a potential fall to the concrete sidewalk below.
