On or about May 25, 2017, the employer located at 16024 Cty Rd X Kiel, WI 53042 did not assess the workplace to determine what PPE is necessary to protect employees from the hazards associated with assembling lights for water fountains. Hazards include, but are not limited to the following: 1. Skin sensitization from components in B-072 Hardener. 2. Skin irritation from components in B-094 Hardener. 2. Respiratory irritation from components in B-072 Hardener. 3. Eye irritation from components in B-094 Hardener and non-ionizing radiation from LED light assemblies.
