Instances included:  a - The employer did not implement the documented Line Break policy and an employee was exposed to Ammonia when no personal protective equipment was donned for that event.   b - The employer did not develop specific written Lockout/Tagout procedures for employees replacing a portion of a 6 in. HSD line on an active Ammonia refrigeration system.   c - The employer did not provide locks to Authorized employees performing service/maintenance work on an active Ammonia refrigeration system.  d - The employer did not provide training in a Lockout/Tagout program and specific written procedures to employees performing service/maintenance work on active Ammonia refrigeration systems.  e - The employer permitted service tech employees to use hand held Acetylene torches to defrost ice from Ammonia lines that were to receive service and/or maintenance.
