Throughout facility - on or about May 14, 2017, the employer did not provide adequate hazardous energy control lockout/tagout training to authorized, affected, and other new hire employees engaged in servicing tasks on industrial machines, to include all of the elements in 29 CFR 1910.147(c)(7)(i)(A) through 29 CFR 1910.147(c)(7)(i)(C). Employees were exposed to machine's unexpected re-energization caught-in hazards likely to result in serious injury.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures)was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Production area, Line 6 - On or about May 14, 2017, prior to starting work, employees (temporary and full-time) required to clean "chrome rollers" at forming rolls and calenders were not required to apply energy controls in the following sequence:   (a) turn off and shut down the machines, (b) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (c) affix lockout or tagout devices to each energy isolating device, (d) verify that isolation and deenergization of the machine or equipment have been accomplished.  The movable barrier guard did not provide effective alternative protection. Employees were exposed to caught-in hazards likely to result in amputation of hand(s) and fingers.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
