29 CFR 1926.451(e)(1): Portable ladder, hook-on ladder, attachable ladder, stair tower, stairway-type ladder, ramp, runway, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used to access scaffold platform more than 2 feet above or below a point of access:  a)  At the job site located at 625 E. Shadburn Avenue, Buford GA, on or about May 25, 2017, an employee was observed working/walking from a fabricated frame scaffold putting up a brick wall.  The employer did not provide an accessible ladder on the scaffold, exposing the employee to fall hazard.

Shadburn Avenue, Buford GA, on or about May 25, 2017, the employer did not provide hazard communication training for an employee who mixes chemical products, such as but not limited to cement/mortar containing silica, which exposed workers to skin  and respiratory irritations .
