Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 5/12/2017, employees from Pennington Roofing Company were removing shingles from a roof of a residence located at 238 Montecito Terrace in St. Peters, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.   The crew lead and two employees were observed on the roof of the building approximately seventeen feet above the ground without fall protection, exposing them to a fall to the ground below.

On 5/12/2017, employees from Pennington Roofing Company were removing shingles from a roof of a residence located at 238 Montecito Terrace in St. Peters, MO.  The ladder the employees used to access the roof was not extended three feet beyond the landing or edge of the roof.
