29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person:    a) Premier Hospitality at 11401 Via Varra Rd, Broomfield, CO 80020:  On and before May 31, 2017, the employer did not ensure that the scaffold was designed by a qualified person.  Two employees were welding steel C channel on the carport canopy while working on an elevated platform connected to a Gehl RS8-44 rough terrain forklift.  The platform was not secured to the forks, and no fall protection system was in place.  This condition exposed the employees to a fall hazard of approximately 18 feet.
