29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:    Worksite: Where employees were conducting roofing operations in excess of six (6) feet above ground level, they were not protected against falls to the ground below by guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    Worksite: When the employer used a portable ladder to access a roof during roof shingling activities, the ladder's side rails did not extend at least three (3) feet above the upper landing surface of the roof.
