29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: On or about June 1, 2017, employees were exposed to respiratory stressors associated with being required to wear 3M half-face and/or full face tight fitting respirators while cleaning up brass shells and lead residue inside gun ranges and the employer did not provide medical evaluations to employees prior to respirator use. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting respirator is fit tested prior to initial use, whenever a different respirator facepiece (size, style, make or model) is used, and at least annually thereafter. On or about June 1, 2017, employees were exposed to respiratory stressors associated with being required to wear 3M half-face and/or full face tight fitting respirators while cleaning up brass shells and lead residue inside gun ranges and the employer did not provide fit testing to employees prior to respirator use. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead: a) On June 1, 2017, a wipe sample was conducted by OSHA revealed the sink handling inside the kitchen area showed a lead surface contamination of 3646.44 micrograms per square foot (ug/ft2). b) On June 1, 2017, a wipe sample was conducted by OSHA revealed the punch time clock inside the kitchen area showed a lead surface contamination of 3265.45 micrograms per square foot (ug/ft2). c) On June 1, 2017, a wipe sample was conducted by OSHA revealed the chair inside the large training area showed a lead surface contamination of 2608.72 micrograms per square foot (ug/ft2). d) On June 1, 2017, a wipe sample was conducted by OSHA revealed the table inside the medium classroom showed a lead surface contamination of 2375.53 micrograms per square foot (ug/ft2). e) On June 1, 2017, a wipe sample was conducted by OSHA revealed the white table inside the large training room showed a lead surface contamination of 765.80 micrograms per square foot (ug/ft2). f) On June 1, 2017, a wipe sample was conducted by OSHA revealed the door handle leading into the gun range showed a lead surface contamination of 654.69 micrograms per square foot (ug/ft2). g) On June 1, 2017, a wipe sample was conducted by OSHA revealed the refrigerator handles showed a lead surface contamination of 640.10 micrograms per square foot (ug/ft2). h) On June 1, 2017, a wipe sample was conducted by OSHA revealed the kitchen table showed a lead surface contamination of 243.96 micrograms per square foot (ug/ft2). Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). Lombard Gun Shop & Range, Ltd. dba Article II Gun Range was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.1025 (h)(1), which was contained in OSHA inspection number 1236550, citation number 1, item number 1 and was affirmed as a final order on March 23, 2017, with respect to a workplace located at 250 Cortland Ave, Lombard, IL. 60148.
