29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways that enabled each employee to recognize hazards related to ladders and stairways, and that trained each employee in the procedures to be followed to minimize these hazards:   a) Job site, 9070 St. Simons Way, Pittsburgh, PA: On or about May 31, 2017, during construction activities, the employer did not ensure that employees were provided with ladder safety training. Deficiencies noted during the inspection included, but were not limited to, working from inappropriate positions on the ladder such as the step below the very top of the ladder or the rear ladder bracing.
